II. Background of your School/Institution Include all facts, with numbers.  Provide the following details ·         number of students; typical parental social level and income;·         number of teachers; average teacher age; average teacher training; salary; ·         if teachers receive annual retraining; provide a little detail;·         when school started, how many classes; how many class rooms; how much building space; play ground space; facilities available in the school (e.g. computers, internet, laboratory, sports, music, library…); how facilities are managed; how these are funded;·         decision processes in school; e.g. does the HM make all decisions? Describe.·         school budget over past 5 years; where do the funds come from,--- if State or city funds are received, a breakdown of funds spent on salaries, facilities etc. for past 5 years;·          write if students are paying anything (and if yes, how much) ;·          are teachers contributing; if yes, how much?·          does the school raise funds in any other way; if yes, how?·          the achievements of students in relation to other schools; ·         extra-curricular activities available, and achievements (awards, competitions)..·         percent of students passing final exam such as SSC over past 5 years (we want to see if you are improving), ·         where do you get water, what is the quality of drinking water; percent of students sick on average day; percent of teachers sick on average day; ·         do you have a pump for water, a water tank? ·         How much do you pay for electricity….·         Are you receiving funds under Sarva Shiksha Abhiyan or other government programs? If yes, how much and how are they being used?·         Have you selected a teacher to represent your school at the VHF Teachers Center? Please provide information on him/her. Present as much information as possible so we can understand your school. Include a few photos of outside, grounds, class-rooms in the pages of the proposal. Be honest, no partial truths. This section should be 2-3 pages long and must be highly informative. III. Proposed School Development Program Describe here how you see your school to have changed at the end of 2, and 5 years. Again, please refer to Part II for what activities VHF will consider funding. This section should not be too detailed. Be specific, use numbers when you can. For example, here is what a school might say:  “We intend to upgrade the school appearance, start a sports and music program, install 10 computers and set up a library of 500 books. Along with this, we intend to improve our administrative set up along the lines suggested by VHF”…Do not make claims that are unrealistic, as we shall see if you do accomplish these goals annually. IV.             Program Implementation Plan This is where the bulk of your details will be. You must describe in detail what the current status is, and how you will change it. This section should contain subsections for each of the Plan components. In each case, write the name of the person who will lead this component and will be manager of this component, and how much money is requested for this component. Describe then how you will sustain this program, ie. How will money be raised to continue the program? For example, here are two components: v.1  Clean water Project ( Program Manager: Indra Vaisakhi; Budget Requested: Rs. 18,652). The present supply of water is from the Ganda Nallah which makes for unhealthy situations. 20% of children are sick on any one day, and teachers too are known to suffer from illnesses due to water. The diseases which are known to us due to poor water are ……The plan is to install a filtration and solar water purification unit. We have studied the systems available on the market and compared their capabilities and prices. Based on this, we have jointly voted to select Pure Water Corporation’s Model Ultra Pure 200…..The system shall be installed by the manufacturer within 1 month of payment. It will produce 200 liters of pure water for drinking purposes each day….Parents will be asked to contribute ¼ Rupee per day for their child’s clean water. v. 2  Music Cultural Program (Program Manager: Smt Sangeeta; Budget Requested: Rs. 48,754) .  The school has no music program at the time of this writing. The reason for this is an absence of awareness, unavailability of funds, and lack of initiative from the school to put emphasis on music education. The result is that students are unfamiliar with our rich classical music, and only understand filmy music to be music. To cure this, we are proposing the establishment of a music program. The program shall have two components: vocal music, and instrumental music. A teacher for the program has been identified. The teacher’s qualifications are provided below….120 students, or roughly 25% of the school shall receive some musical training. The plan is to provide this instruction after school hours….5 hours per week…the teacher salary will be ….,  We have requested Rs. ….for purchase of musical instruments. A breakdown of this amount is provided below. However, if these funds are not available, we may be able to rent the instruments for Rs….per year, or finance the purchase over 3 years….At the end of the first year; the school shall enter competitions …. In each case, write the proposal so that it is clear to Vishal Himalaya Foundation that you are capable of designing this program with the best ideas from all students and teachers in your school, that your school has achieved significant results in prior efforts, that the persons who are program managers have the capability to succeed, and you must convince us that you have done your homework in coming up with the best budget for the plan. V.                Matching Funds Describe here how you are raising Matching Funds, how much, and from which organizations or donors. As has been noted, Vishal Himalaya Foundation believes that any organization that requests funds should raise a part of the funds locally. Doing so shows the ability of the requesting organization to achieve specific goals. The greater the part of funds a school/organization raises, the more likely it is to win a grant from VHF. For example, of 2 proposals for libraries from two schools, one for Rs. 50,000 and one for Rs. 25,000, the more expensive proposal could win if they raise a higher percentage of funds themselves. Other factors may play here too, e.g. a slum school with a lower percentage may also win. As was described in the PROGRAM, those schools who raise funds shall receive high priority in the order listed therein. VI.             Management Plan Describe here who will be Chief Program Manager (CPM), and how passionately this person has already worked to improve education. Note if he/she has any experience in management of projects, how this person was selected, and how much support this person can expect from the staff and school management. VHF requires that the Chief Project Manager must be selected with the consent of the teachers and parents (if possible). Please take great care to convince us that you have a sound management plan, and that you will monitor your own progress. Please state that the CPM has an e-mail address and knows how to use it.  VII.          Budget   Include a reasonably detailed budget. We are looking to see if you have carefully considered the costs and if you have done your homework.           (see an example budget below; get prices of items first)           Salaries    (3 months at Rs.8,000 per month)                          Rs 24,000         Transportation costs                                                                Rs.   2,000         Materials and Equipment                     TV (Rs. 15000), Books (Rs.10,000), Paint, Posters,                     Water pump,   plants, etc.                                           Rs. 30,000            Other Costs (provide details)                                                  Rs.  5,000                                                             TOTAL COSTS:                              Rs. XXXX.            Matching Funds from Parents            Rs. 5000            Matching funds from city Donors       Rs. 10,000            Funds requested from SSA                Rs. 20,000            Loan from Generous Bank                 Rs. ….                                                            TOTAL MATCHING FUNDS       Rs. Yyyyy Funds Requested From VHF (difference of TOTAL COSTS – TOTAL MATCHING FUNDS)         Rs. XXXX
